There is not a dedicated app for mobiles. You can access the HazView reporter through the URL (Type https://oce.hazview.com into the address bar of your internet browser).
Mobiles will allow you to save this as a bookmark and add it to your homescreen for quick access.

The best user experience is on a computer.
